In Section 20, item 3, Experience and Availability of Key Personnel, requires a Design Quality Control Manager and Construction Quality Control Manager as key management personnel. In Section 20, item 4, Personnel by Discipline, is requiring a design quality control and assurance and construction quality control and assurance as non- key personnel. Are these positions in section 20, item 4, Personnel by Discipline a duplication from what’s stated in Section 20, item 3?

Referencing Section 20, Item 3, Experience and Availability of Key Personnel; The Construction Quality Control Manager and the Design Quality Control Managers are considered key personnel and their individual qualifications, expertise, and experience will be evaluated relative to the scope and complexity of this project. Referencing Section 20, Item 4, Personnel by Discipline; The qualifications for each of the critical disciplines listed in Section 20 Item 4 will be evaluated based upon the work flow, interrelationships among teams and their demonstrated ability to function as an integrated Design-Build team.

With reference to the Department's pre-proposal meeting on July 23rd, it is our understanding that the Department will be responsible for completing the NEPA process and filing with FHWA to support the BTC, and will subsequently issue status and/or findings as part of the RFP documents. Please provide a list of all other individual permits that are anticipated to be required by this Project, and to what extent the D/B Entities are responsible for securing such permitting.
Answer:
A list of anticipated permit documents required to construct the BTC (other than NEPA, which will be completed by RIDOT) will be provided with the RFP package, but it will be the responsibility of the chosen D/B Team to confirm the complete list of required documents for their proposed design, submit the necessary documentation, and obtain all required approvals.

Will the Department be releasing a complete survey basemap of existing conditions with the RFP, or will this be the responsibility of the D/B Entity?

In response to our question submitted on 7/16/19, RIDOT indicated the JV “must register with RIVIP and provide the Bidder Certification form in the SOQ.” Registering with RIVIP requires that the JV provide a taxpayer ID, which requires requesting a W-9. Page 9 of the RFQ stages “Joint Venture firms are not required to register with the Secretary of State as a JV, nor are they required to submit a W-9 as a JV during the SOQ selection Phase 1. This will be a requirement of the successful Respondent only.” Please clarify whether the JV must request a taxpayer ID number in order to bid the project. If not, should both JV partners submit their individual RIVIP forms?
Answer:
The JV is NOT required to request a Taxpayer ID number in order to submit a Statement of Qualifications (Phase 1). For the purposes of the Rhode Island Vendor Information Program (RIVIP) it is acceptable for the Lead Contractor to register under the Joint Venture name but include their (Lead Contractors) Tax Payer ID Number since payments will not be made under Phase 1.

Can resumes (RFQ requests at least 16 resumes) and project descriptions (potentially 16 as well) be included in an appendix and not contribute to the page count?

Section 19, Item j (page 15 of 24) requires the Lead Design Firm and engineering subconsultants to submit an SF 330. We request that this requirement be deleted entirely as it duplicates information required elsewhere in the RFQ. Alternatively, we request that the requirement be modified to require only SF330 Part II, not SF330 Part I. SF 330 Part I is customized to present a design team’s qualifications for a specific project. SF 330 Part II provides an annual snapshot of employee discipline composition and annual revenues by project category for each firm involved in the design team. By requiring SF 330 Part I and the information requested in Section 19, Items a thru v, RIDOT will receive the same information related to the design team in two different formats. The information in SF 330 Part I generally duplicates material requested in previous items of the RFQ.

If the information requested in a particular section of the SF-330 (e.g., resumes) is included elsewhere in your Statement of Qualifications, please reference that Section on the SF-330 rather than duplicating information.

The Statement of Qualifications Submission Requirements require the lead construction entity and the lead design entity to list a maximum of 8 projects completed within the past five years to best illustrate each firm qualifications. We request that the period of the projects completion be increased from five years to at least 15 years in order to better qualify the projects of similar size and complexity.

The Lead Construction entity and Lead Design entity shall list a maximum of 8 projects completed within the past fifteen years that best illustrate each firms qualifications most relevant to this project scope with a focus on the ability to deliver such projects on schedule and on budget.

The Required Forms Matrix in Section 14 requires the Conflicts Disclosure Form "for Board of Directors, key personnel or those with a potential conflict to disclose." The companies making up the D/B Entity have a large number of members on the Boards of Directors. Must this form be signed by all members or is one form from each Entity sufficient?
Answer:
Anyone in your organization who will be assigned to this project and has a potential conflict should submit a Conflicts Disclosure Statement. If, upon confirming with the Board of Directors/employees it is determined that no conflicts exist, it is permissible to attach a list of applicable Board members/employees to the Conflicts Disclosure form. The form shall be signed by an authorized representative of your firm. If an individual is unsure whether a relationship, association or connection constitutes a Conflict, please consult with RIDOT's Legal Office at 401-222-6510

Section 14 states that forms must be submitted in the SOQ package by EACH D/B TEAM ENTITY, but the Required Forms Matrix on Page 13 does not require any forms from the Lead Engineering Firm. Section 3 states that the D/B Entity consists of a Lead Contractor and its key subcontractors, one of which is the Lead Designer. The Required Forms Matrix does require forms from subcontractors. Please clarify which forms must be submitted by the Lead Designer.

The Lead Engineering firm must submit the Debarment, Lobbying, Conflicts Disclosure, Certification for Title VI Assurance and the DBE Special provisions forms.

The key personnel are as follows:Project Manager, Superintendent, Design Manager, Construction Manager,Design Quality Control Manager, Construction Quality Control Manager & QA/QC Administrator. Section 19 Part i. is revised as follows: Resumes for all key management staff proposed including the Project Manager, Superintendent, Design Manager, Construction Manager, Design Quality Control Manager, Construction Quality Control Manager, and QA/QC Administrator. In addition, provide resumes for the individuals proposed to manage the following functions: Project controls, subcontracts and procurement, Subsurface Utility Engineering/Coordination, Environmental compliance and mitigation, Safety, Labor Relations, Geotechnical investigation, survey, or any other position which you deem is essential to the success of the project.
